有很多学者对不同年龄阶段儿童的血清CysC水平进行测定,得出的结论是CysC不通过胎盘,因此婴儿血液中的浓度不受母体水平影响[13]也不受肾前性因素及脱水状态的影响[14],CysC在早产儿血清中的浓度高于足月儿,并且CysC浓度与胎龄、体重、血红蛋白、性别、胆红素水平无关。整个儿童时期出生时浓度最高,随后的几个月浓度逐渐下降。国外多位学者测定的不同年龄的参考值如表,测定方法运用免疫比浊法。

5基于CysC的肾小球滤过率公式的临床应用 CysC的检测目前可在自动生化分析仪上进行,适合于临床常规应用。CysC的测定在反映肾小球滤过率方面有很多优势,对各种慢性肾病的早期诊断有重要作用。但基于CysC 的肾小球滤过率计算公式的应用尚未成熟。CysC要做为一种理想的GFR标记物替代肌酐还需进一步的论证和临床应用研究。 6问题与展望
综上所述,由于CysC特定的生物特性及功能,使其成为比较理想的肾小球滤过率的标志物。但在临床实际应用中,还存在一些问题:首先是测量方法有待完善以进一步提高其准确性和精确性,第二是如何排除一些影响因素的干扰,第三CysC的价值还需要一些大规模、多中心和前瞻性的试验提供足够的数据支持。相信随着对CysC研究的深入和其本身测量方法的改进,CysC必将显示出越来越重要的临床价值,为我们更好的了解肾脏病的发病机制和早期诊断提供很好的帮助。
